Please fill out the following information to get your membership
in the MA4 Birthday Club started. Please note that it takes
approximately two weeks to get your birthdate registered.
Registered members will receive a "Happy Birthday" email
informing them that their "Birthday Certificate" is ready and
available for use for up to 10 days before or after their birthday.
When you come in to enjoy entrees with at least 3 other guests,
your "Birthday Certificate" is good for the cost of your entree*

PLEASE NOTE:
We will need your first and last name, birth month and day as well as your email address. (Don't worry we aren't asking for the year). Membership in our Birthday Club enrolls you in our occasional email blast (sent in a suppressed recipient format). Should you unsubscribe from our mailing list, you may be removed from the Birthday Club.
The Birthday Club Certificate is not valid in conjunction with any other offer (including dining awards or in-kind trade certificates), on holidays, or during prix-fixe or special events. Only one birthday club certificate may be used per table/reservation.
If you registered for the club but fail to receive your Happy Birthday Email, please give us a call. It is most likely that we have your name and birthday, but an error was made in entering your email address.
*In the event that all guests do not order entrees, we reserve the right to determine what food item, if any, for which the Birthday Certificate will be used.
